Registration Procedure and Fees

All policies regarding registration fees are set by Creative Solutions Unlimited, Inc. (CSU), and are reviewed periodically. These fees include all of the following: seminar handouts, computer lab fees, recording of attendance, and CEUs. Supplemental textbooks and reference books are not included in the fee. All attendees, including ULO subscribers, must complete the written registration form. There will be a $20 charge for registrations received after the subscription fee. Non-subscribers are required to submit payment prior to seminar. Attendance at seminars is limited to adults only. CSU reserves the right to cancel any seminar that does not meet registration requirements. The fee structures will be specified on the brochure for each seminar. CSU does not discriminate in its programming based upon race, religion, color, creed, sex, national origin, age, physical or mental disability, in accordance with Section 504 of the Rehabilitation Act of 1973, and Title IX of the Education Amendments of 1972.

Telephone Registrations

CSU will not accept telephone registrations for education seminars. For acceptance, registrations must be mailed to the CSU office, sent via FAX to (641)892-4475, or click Registration to submit on-line.

Refunds

The ULO 13-month subscription fee is non-refundable. Refunds to non-ULO subscribers for a specific seminar will be granted to those persons who notify the CSU office BY THE CANCELLATION DATE. There will be a $5 processing fee on all refunds. Refunds WILL NOT be granted to those persons notifying the CSU office after the cancellation date for the seminar. NOTE: IF you discover after the cancellation date that you are unable to attend a specific seminar, you may contact CSU to transfer your registration to another person at your facility, sending them in your place.

Program Cancellations

CSU reserves the right to cancel any education program. All registration fees paid by non-ULO subscribers will be refunded in full. Every effort will be made to reschedule ULO subscribers into an equivalent CSU seminar.

No Shows

All no shows will be billed; substitutions are welcome. Any ULO-subscriber who pre-registers for a seminar and fails to cancel prior to the cancellation date for the seminar or fails to attend will be assessed a $25 administrative fee. Payment of this fee will be necessary before any additional registrations will be accepted for the subscribing facility. Any non-ULO subscriber who pre-registers for a seminar and fails to cancel prior to the cancellation date for the seminar or fails to attend WILL NOT receive a refund. NOTE: If you discover after the cancellation date that you are unable to attend a specific seminar, you may contact CSU to transfer your registration to another person at your facility, sending them in your place.

Cell Phones/Beepers/Taping

As a courtesy to both attendees and faculty, CSU requests that cell phones and beepers be shut off while a seminar is in session. Material presented in the seminar is the property of CSU. Taping of the session is not allowed.

CEU Credit

Each seminar brochure will designate the number of credit hours and for whom those hours are approved. CSU awards CEU credits upon attendance of all sessions of a program and the completion of an evaluation. 100% attendance is required to verify satisfactory completion before credit is awarded. Seminar doors will close promptly 15 minutes after the designated beginning time of the seminar. No one will be admitted after that time. Partial CEU credit is not offered and will not be given. Breaks are given in each education seminar of two (2) hours or more in length. Failure to return to the room at the conclusion of the break will result in denied credit for the session. CEU certificates or certificates of attendance will be presented following the seminar.

CEU Credit Records

Records of CEU credit will be maintained with the contracted provider of the CEU credits for a minimum of four (4) years from the date of the seminar. Copies of CEU forms can be obtained by contacting CSU with the name and date of the program, and the name and title of the attendee. There will be a minimal charge due prior to the copy being sent.

Registration Deadline/Cancellation Date

Registration deadline and cancellation date is five business days prior to seminar.
